Summary

Come and help us improve our heart health services for Hertfordshire residents.

Detailed description

Our Heart Failure Team in Hemel Hempstead, Hertfordshire offer care for local residents in need. The service are keen to better understand what their patients like about the service, and what could be better for them. 


Come and join the team and engage patients to tell us about the impact their care has had on them so we can learn and improve the service. 


  • As part of the team you’ll be asked to contact a list of patient (names and contact numbers will be provided) and ask for feedback using a pre scripted questionnaire – Minesota.
  • You will be asked to write the patients feedback on a template that will be provided (Microsoft Forms)
  • To keep a record of those patients who you were not able to contact.
  • To liaise with your role manager if any concerns are raised during the feedback call with patients
