Ticket Home is the UCAN home from hospital scheme helping patients to safely be discharged from hospital and return home.
As a Ticket Home from Hospital volunteer, you will provide social, emotional, and practical support to a patient being discharged from hospital. This will improve their emotional health and wellbeing and help them feel less socially isolated.
You will be supported by our Ticket Home Community Hub Coordinator and will be given training including: Your role in the Ticket Home Project, Safeguarding, Confidentiality, Lone Working, Health & Safety.
As a volunteer, you will assist with such things as:
- Helping with shopping
- Picking up prescriptions
- Making wellbeing calls to check in with the patient
- Providing Information about other services
- Or even just having a chat
