Summary

This role involves researching local community groups and services so that our Living Well nurses have a list of services that patients can be referred on to.

Detailed description

Our co-ordination centre is the central hub for all patient referrals to the wide range of services that RGP offer. We are setting up a central resource of other local services and community groups that patients who are not suitable for our services can be referred to. Initially we are looking for some help collating information from our own professionals and contacting external organisations to understand what services they offer. Once the central resource is in place, we will need ongoing support to keep it up to date

 

Main tasks

sourcing information about external resources, community groups and services • understanding the referral criteria for each service • establishing a current resource list of groups, services, contacts and referral criteria for the co-ordination centre and our nursing and medical teams to use for sign posting patients • updating the resource list regularly with any revised information, referral criteria, new services/groups • maintaining good relationships with local organisations and community group contacts


What are we looking for?

• good IT and internet research skills

• good communication skills with the confidence to approach RGP professionals and external organisations

• the ability to use your own initiative and liaise with community organisations

• someone with knowledge of local community and support services/organisations is desirable

• someone who is reliable and flexible

About Rennie Grove Peace Hospice Care

At Rennie Grove Peace Hospice Care, our unique care provides a lifeline of compassion and support to patients, carers, and families facing progressive life-limiting illness.

We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.) Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces.

We are proud to offer care in the place that best suits the individual’s needs, whether that’s at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities. As a hospice care charity, we have offered this support, free of charge, for over 40 years.
