About Central London Community Healthcare NHS Trust

CLCH provides community health services to more than four million people across 14 London boroughs and Hertfordshire and commissions continuing health care across West and North London. CLCH is a prospective advanced foundation trust.

Our vision is to deliver great care closer to home.

Our mission is to enable thriving communities that give our children the best start in life and adults greater independence.

Every day, our staff provide high-quality community healthcare in people's homes and local clinics, offering more than 105 clinical services, helping people to stay well and independent in the place they call home, manage their own health with the right support, avoid unnecessary trips to, or long stays in, hospital.

We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, physical rehabilitation and palliative care for people towards the end of their lives.
